Eccrine porocarcinoma (malignant eccrine poroma) is a very rare malignant tumor of eccrine sweat glands origin. This type of tumor is usually connected with a good prognosis, however, approximately 20% of cases develop regional metastases and in 10% of patients distant metastases are diagnosed. As the eccrine porocarcinoma represents an uncommon tumor, the optimal treatment remains to be
determined. Here we report the case of a 53-year old patient, underlining the diagnostic and therapeutic difficulties of metastatic eccrine porocarcinoma.

Specifically, we want to point out the unusual localizations of the metastases due to hematogeneous dissemination (mouth cavity, external auditory canal, scrotum, bones, cavernously changed voluminous lung metastasis), and unfortunately the nearly absolute resistance of this extremely uncommon tumor type to both chemotherapy and radiation therapy.
