Summary: The authors offer review and critical assessment of published data concerning pelvic exenterations. Those are standard procedures in onco-surgery. The most frequent indications are recurrences or progressions of cervical, vulvar or vaginal cancers. A basic condition for the procedure is an absence of distant metastasis. Currently presented mortality reaches 5 % to 10 %, early and late postoperative morbidity is frequent (40- 60 %). Supralevator type of procedure might be followed by low colorectal anastomosis and ortotopic pouch creation using urethra for urinary diversion. The most significant arguments justifying the procedure are favorable followup results – 5-years overall survival reaches 50 – 60 %. Early diagnosis of recurrence and early procedure are fundamental for both procedure outcome and prognosis.
