Optimalization of dosis distribution in radical locoregional radiotherapy of breast carcinoma

Summary: The authors describe a modified technique of locoregional radiotherapy in patients with breast cancer. Modern equipment is usedmulti-leaf colimator (MLC), asymmetrical fields and full 3D planning system. The breast and chest wall are irradiated through usual tangential fields. With the same isocentre, axillary and supraclavicular lymphnodes are treated through three fields. Identical dose into both of the lymphatic drainage areas is achieved by the combination of two anterior fieldsone open, the other with a MLC block exactly corresponding with the shape of the dorsal axillary field. In comparison to classical „filling-up“ of axilla through an additional dorsal field, which regularly causes overdosing the ventral parts of axilla, the presented technique achieves a very satisfactory dose homogeneity. The additional planning-time is minimal.
